Either complete multiple G-1450 Forms or request separate checks...the H-1B petition will be rejected if the fees are combined in one G-1450 or check.

If you are paying a fee with a G-1450, do not list the applicant's name on the form. Instead, list "Temple University" in the "Given Name" field and leave "Family Name" blank.

Ordering University Checks

All Checks should be made payable to The US Department of Homeland Security.  The current address is USCIS, California Service Center, 2642 Michelle Drive, Tustin, CA 92780.

University Checks for all Non-immigrant Petitions should be ordered through Concur (use TUMarketplace if you do not have permission to submit invoices in Concur ).  Some areas of the University may be required to submit additional forms as indicated by their department. Use the search grid on the left side of the screen to pull up the US Department of Homeland Security as the vendor. Select Special Handling for H1-B Visa from the drop-down list and check the box for Direct Deposit Override .  Leave the description field blank. Include a screenshot of both this webpage and the front page of the DestinyOne application with your check request. The name of the Applicant must be visible. Since there is no traditional invoice with an H-1B check request, make sure you use the payment request form. The bank will send the checks to ISSS via UPS.  ISSS will staple the checks to the petition and then mail it to USCIS. ISSS will forward any checks for O-1 Petitions that we receive to the attorney working on the case.

Make sure that you include the Accounts Payable Payment Request as you will not have an invoice for the checks. This form will open as a Word Document.
